Hello DUX Families,
As we approach the start of August and the scheduled start for fall sports, I want to provide you with some updates and reminders. On July 29th, the MHSAA announced their intention to move forward with fall sports with modifications for each sport’s return. The coach of each sport will communicate on the modifications for their programs return to play protocol. You can also find them linked below.
As of right now, the start dates for High School sports are as follows:

Register Your Athlete for Sports
The best way to stay informed about tryout/practice times, requirements, and additional expectations is to have your student registered in FinalForms for the sport they are interested in playing. Each sport’s guidelines for return to play are unique so it is incredibly important that your athlete is signed up for their sport in FinalForms. Sport specific communication will come exclusively from coaches via FinalForms.
In addition to providing information and signing the required forms, to complete the FinalForms registration you will be asked to provide either a sports physical or a Health Questionnaire.

Due to COVID-19 the MHSAA is allowing sports physicals from the 2019-2020 school year to carry over to 2020-2021, with the competition of the new MHSAA Sports Health Questionnaire. When you register for 20-21 in FinalForms, this will be one of the forms in the list. You can electronically fill out the form on FinalForms, print it off and submit it to the athletic office. Please make sure the completed form goes to the school you will be attending in 2020-2021.
If you did not have a valid sports physical in 2019-2020 then you must complete a full sports physical with an MD, DO, NP or PA in order to be eligible to participate in 2020-2021. This year, due to COVID-19, we are accepted emailed or faxed physical forms and health questionnaires. Please email them to email@example.com or firstname.lastname@example.org.
Athletic Department Hours
The ZWHS Athletic Department will be open Monday-Thursday from 8-2 beginning August 3rd. We will also accept electronic versions this year due to COVID. Please send your clear PDF file to email@example.com OR firstname.lastname@example.org.
The Creekside Office will be open Monday-Thursday from 8-3 beginning August 3rd. Creekside forms should be sent to: email@example.com. Please turn in Creekside forms to Creekside!
All prospective 2020-2021 student athletes who do not have a current, valid baseline ImPACT test should plan to take a baseline test prior to their sports season. A current baseline test is one that has been completed in the last two academic years beginning in high school. All incoming 2020-2021 freshmen, new or transfer students will need to complete a test. All 2020-2021 juniors and anyone who does not have a current test will also need to test.

Athletic Passes: Information will be made available once we have more guidance on spectators.
